Community Harvest helps individuals and families build healthier lives through prevention-focused education, food-centered wellness, Community Health Worker support, and connection to trusted community resources.

Why we exist

Health is shaped by more than a doctor’s visit.

Food access, stress, social connection, transportation, work schedules, trusted information, cultural belonging, and the support people have around them all shape health.

For many adults—especially those living in resource-limited communities—the path to wellness can feel fragmented and overwhelming. Community Harvest brings people back to the table, literally and figuratively, so they can learn, connect, share resources, and build healthier habits together.

Bring prevention-focused wellness into a trusted community space.

Clinics, public health organizations, schools, faith communities, employers, housing communities, funders, food partners, and local leaders can host a cohort, sponsor meals or materials, refer participants, support CHW outreach, or co-design community wellness initiatives.
