Community Health Workers are trusted bridges between communities and the systems that serve them. Our CHW-centered model strengthens trust, improves communication, supports resource navigation, and helps participants receive guidance from people who understand the communities they serve.

Community Harvest equips community health workers with the tools to transform local health outcomes by investing in their professional development and providing training, leadership skills, bilingual and culturally responsive materials, and outreach planning.

Community Harvest highly values CHW program models as evidence-based frameworks that have documented effectiveness in improving access, outcomes, and community engagement. We believe in appropriate CHW compensation, training, and support. When funding allows, paid paired CHW outreach will help ensure participants receive guidance from people who understand local communities, cultures, and barriers.
